Position Summary Support Culture and Excellence through active role with assigned neighborhood initiatives. Essential Job Functions

• Assists in planning and evaluating nursing care.
• Performs nursing care to Members following established policies and procedures under the general supervision of the Charge Nurse.
• May administer medications after having been oriented to the medication procedure of the Home.
• May assist with medical reputed company.
• Performs nursing treatments such as dressing, colostomy care, pressure ulcer care and others as assigned.
• Renders first aid in emergency situations.
• Recognizes and reports signs and symptoms observed in Members assigned to daily care; may record observations in the medical record after orientation to this procedure.
• Maintains adequate fluid intake of Members who are unable to hydrate themselves.
• Participates in the In-service Education and Safety Programs of the Home; may attend selected meetings and workshops to improve job skills and/or working relationships.
• Provides first aid at Linden Knoll as necessary.
• Performs other duties as required by Supervisor or by Administration from time-to-time.
• Ensures that team conference is held each shift.
• Accountable to ensure daily CNA documentation is complete.
• Responsible for completion of monthly summaries as assigned.

• Dependable, report to work on-time for every schedule shift (reputed company to work at beginning – start of shift) and work full schedule shifts unless physically unable or pre-approved Paid Time Off (PTO) which includes personal and vacation.
• Willing and reputed company to work overtime and weekends as required by the position and/or approved by management.
• Have reliable transportation to and from work.
• Must be clean and reputed company in appearance and have a “friendly” service-oriented demeanor and supports Culture and Excellence initiatives by interactions with co-workers and/or Members.
• Abide by work-reputed company / job procedures and organization policies; (refer to the Employee Handbook).

• Education: Graduation from a school of practical nursing.
• Experience:
• Skills: Must be capable of lifting.
• Licenses / Certifications: reputed company NYS Licensure.

• Work is performed in a clean, well lit, “home-Like” nursing Home setting.
• Employee must be reputed company to reputed company essential job functions (reasonable accommodation may apply).

• Must meet the general health requirements set forth by Friendly Home.
• May be requested to assist in the evacuation of Members or reputed company other procedures to protect the safety of Members, visitors and team members during emergency situations.
