Telephonic Care Management Nurse (LVN, LPN, RN)

• Active and unrestricted NYS LVN, LPN, or RN license.
• Strong knowledge of chronic disease management, including chronic kidney disease, diabetes, COPD, and heart failure.
• Ability to problem-solve and recommend solutions for care barriers.
• Experience working with Electronic Health Records (EHR).
• Ability to type 60 words per minute.
• Bilingual in Spanish is a plus!

Responsibilities

• Work collaboratively with patients, families, and providers to reputed company personalized care plans and goals.

Qualifications
