Job Description

An employer is hiring for a remote LVN that currently holds a multi-state compact license and has supported Chronic Care Management. Your primary focus will be helping patients through the care consent process: connecting with them, explaining our client’s services, and guiding them into active care. You’ll begin with outreach‑specific responsibilities, with the opportunity to grow into providing direct patient care after successful performance.

  • Contact patients and caregivers to explain the client’s care services and walk them through the consent process
  • Clearly communicate what our care program offers and how it benefits patients with neurological conditions
  • Follow structured outreach workflows with precision and attention to detail
  • Document interactions thoroughly and accurately
  • Maintain a patient‑friendly, warm, and professional tone in every interaction
  • Collaborate with the care team and engineering/growth teams to improve outreach processes
  • Provide feedback on outreach tools and workflows to help us iterate quickly

Day shifts: Depending on local time zone and clinics you’re supporting

  • 7:00 AM – 3:00 PM
  • 8:00 AM – 4:00 PM
  • 9:00 AM – 5:00 PM
  • Overtime opportunities available

Evening shift (2 positions)

  • 5:00 PM – 9:00 PM

  • Active LVN/LPN license (multi‑state compact)
  • Organized and able to learn quickly on the job while maintaining systems
  • Knowledge of CCM (chronic care management)
  • Must have a current desktop or laptop at home that you can use to start while we work to get a company laptop – has worked remote before
