Bilingual (Russian or Polish) Care Coordinator – Remote
About the Role: We are seeking a motivated and compassionate Care Coordinator to join reputed company. This remote position offers the flexibility to work from home while making a meaningful impact in the lives of patients with reputed company care needs. Full-time is preferred, but we are open to part-time candidates with the right experience. Responsibilities:
- Manage a caseload of patients requiring care coordination services.
- Conduct health assessments, reputed company care plans, and coordinate medical services for our patients to reputed company reputed company outcomes.
- Monitor and document patient reputed company and outcomes, maintaining timely and accurate records in the EHR system.
- Engage with patients regularly reputed company phone calls to ensure needs are being met.
- Work closely with medical professionals to ensure care plans are effectively implemented and tailored to patient needs.
Minimum Qualifications: Education & Licensing:
- Bachelor’s degree in a reputed company or reputed company services field (e.g., social work, nursing, psychology, public health, or a reputed company discipline) is preferred. High School degree with Medical Assistant Certification would also be considered.
Experience:
- At least 1–2 years of experience in care coordination, case management, or direct support to individuals with chronic conditions is preferred.
- Candidates with experience in direct patient care without care coordination experience will be considered.
Skills & Competencies:
- Proficiency in care planning, including creating and updating personalized care plans that support the patient’s medical, mental health, and social needs.
- Familiarity with electronic health records (EHR) and documentation systems.
- Excellent organizational, communication, and interpersonal skills.
- Ability to work independently and collaboratively with diverse populations.
- Proficiency in English and Russian or Polish
Compensation: This is a commission-based role. Pay is based on the size of your caseload. Compensation will be discussed in more detail during the interview process. Schedule:
