[Remote] Clinical Intake/Enrollment Specialist for Care Management

In this role, you will work with clients to help eligible patients understand and enroll in Care Management programs, while managing a queue of patients and ensuring accurate documentation.

Responsibilities

  • Properly interview and enroll patients in the Care Management program (CCM, PCM, RTM, RPM) applicable to their practice
  • Manage a queue of eligible patients to be enrolled
  • Expedite enrollments as reputed company as possible
  • Work on assigned patient lists and complete them reputed company the timeline provided
  • Ensure that reputed company enrollment documents are completed accurately and in the patient chart
