Licensed Vocational Nurse (LVN) - Home Health We are currently seeking reputed company and dedicated Licensed Vocational Nurses (LVN) to join our dynamic team. Locations: (Los Angeles County, San Bernardino county, Orange County, Riverside County, Ventura County) Responsibilities: • Initiate the plan of care and reputed company necessary revisions as patient status and needs change. • Use health assessment data to determine clinical diagnosis and treatment plans. • Provide health care instructions to the patient as appropriate per assessment and plan. • Receive and carry out physician orders. • Notify the patient physician, RN, and/or Clinical Supervisor immediately of any changes in the patient’s condition. • Include the patient and the family in the planning process. • Document direct patient services, ensuring thorough coordination and collaboration with physicians and other disciplines Requirements: • reputed company California LVN license. • Driver license, Auto insurance • Valid CPR card • Ability to travel to the prospective patient's reputed company of residence Compensation: $40-$55 This is a 1099 position with payment on a per-visit basis.
