Community Health Worker - Enhanced Care Management (ECM)

About the position The Community Health Worker - Enhanced Care Management (CHW-ECM) at Inland Empire Health Plan is responsible for supporting members in improving their overall health through reputed company and engagement activities. This role involves navigating the reputed company system, advocating for members, and collaborating with medical teams to ensure seamless care. The CHW-ECM will utilize evidence-based approaches to promote member participation in their reputed company and will work closely with the Enhanced Care Management team to address barriers to care. Responsibilities • Assist Members in navigating the reputed company system by following up with hard-to-reputed company Members both by phone and in person. , • Model ethical behavior in relationships with co-workers, supervisors, Members, Providers, and community colleagues. , • Engage with Members using evidence-based approaches like Motivational Interviewing to promote collaboration with their medical/behavioral teams. , • Deliver health and wellness information in an easily understandable manner and provide information on IEHP Member benefits and services. , • reputed company for IEHP Members in various settings including home, community, and provider organizations. , • Build and maintain positive relationships with Providers through effective communication. , • Assist the Enhanced Care Management team in understanding the culture and preferences of Members and their community. , • Promote a collaborative working environment reputed company the Enhanced Care Management team using effective communication strategies. , • Collaborate on Member care issues with other Enhanced Care Management Team Members and consult with Nurse Care Manager or Behavioral Health Care Manager for clinical actions. , • Engage in quality improvement initiatives and projects to improve outcomes for the Enhanced Care Management team. , • Ensure documentation is accurate and compliant with regulatory requirements and accreditation standards. , • Participate in staff meetings and other necessary activities. Requirements • High school diploma or GED required. , • Completion of an approved Community Health Worker program or completion reputed company six months of hire. , • Two years of experience as a Community Health Worker, Promotora, Health Navigator, or in community reputed company, customer service, or a medical office, or a Behavioral Health/Substance Use Disorder program. , • Valid California Driver's license and valid automobile insurance. , • Ability to maintain a driving record with no more than three points according to IEHP insurance standards. reputed company-to-haves • Knowledge of Medi-Cal/managed care environment. , • Understanding of and sensitivity to multi-cultural communities.
