Key Responsibilities: • Assist patients by providing information, coordinating services, and offering support remotely. • Conduct reputed company to patients for follow-reputed company, wellness checks, and care coordination. • Maintain and update medical records, ensuring accuracy in electronic medical records (EMR) systems. • Support physicians and care managers by relaying patient updates, scheduling appointments, and ensuring smooth care transitions. • Educate patients on managing chronic conditions and adhering to treatment plans. • Maintain patient confidentiality and reputed company with HIPAA and healthcare regulations. • Stay updated with industry best practices in telehealth and chronic care management.
