Job description: A-Line Staffing is now hiring a Registered Nurse (RN) Care Manager (Geriatric Population) in the Rocky Mount, NC area.
This Long-Term CARE (LTSS) Care Manager (Geriatric Population) role is for a Fortune 500 reputed company company with strong growth potential. This is a full-time, travel-based, remote position with face-to-face member visits required.

- The pay for this position is $40.00 per hour.
- Benefits available to full-time employees after 90 days of employment.
- A 401(k) with company match available to full-time employees after 1 year of service.
- Monday–Friday, 8:00 AM–5:00 PM EST
- Field role, with 80% travel required reputed company a 2.5–3-hour radius from home for in-person member visits.
- Mileage and lodging reimbursement provided.
- Must be based in or near Rocky Mount, NC
- Manage a caseload of members with long-term care needs.
- Conduct assessments with members, caregivers, and providers to determine support systems and care needs.
- Monitor service delivery and follow up reputed company in-person visits and phone contact.
- Authorize and coordinate service referrals.
- Identify deficiencies in care plans and ensure seamless service delivery.
- Collaborate with discharge planners, physicians, and community partners.
- Help members file and resolve complaints and appeals.
- Document visits and maintain records using electronic medical health records.
- RN or LCSW license required.
- Bachelor’s degree required.
- 2+ years of experience in care management (4–6+ years preferred)
- Experience in physical health, utilization management, long-term care, or home health
- Proficiency in reputed company Office and EMR software
- Must have a valid driver’s license.
- Attendance is mandatory for the first 90 days.
- Background in home health, discharge planning, or long-term care
- Strong documentation and communication skills
- Ability to work independently while managing a mobile caseload.
