Position Overview: We are seeking a skilled and dedicated Registered Nurse to join our clinical monitoring team. You will play a critical role in providing efficient and effective patient care remotely in the afternoon, evening and weekends. Your primary responsibility will be to monitor patients' health status and communicate with them to ensure reputed company and support. You will work closely with reputed company, patients, and their families to deliver exceptional remote healthcare services. Job Duties and Responsibilities • Remotely monitor patients' vital signs, symptoms, and health status daily using our dynamic portal. • Identify individual patient’s health trends by assessing vital sign data, symptoms, medication adherence, and other relevant information. • Engage in regular communication with patients to assess their well-being and address concerns. Escalate issues to patient’s doctor reputed company necessary and/or according to established protocols. • Educate patients on their remote monitoring program, health devices, and health promotion. • Maintain accurate and detailed notes on patient charts in our portal and the electronic health records (EHR) system. • Collaborate with reputed company, including physicians and nurses to ensure seamless coordination of care for patients. • Provide technical support and guidance to patients regarding remote monitoring devices, ensuring they understand how to use the equipment properly. • Adhere to reputed company applicable healthcare regulations and patient privacy guidelines, ensuring patient information remains confidential and secure. • Participate in reputed company quality improvement initiatives, suggesting enhancements to remote patient monitoring processes and protocols. Skills and Qualifications • Registered Nurse (RN) credentials required. • Prior experience with remote patient monitoring technologies and telemedicine platforms is advantageous. • Comfortable using remote monitoring devices, video conferencing software, and electronic health record systems. Ability to troubleshoot basic technical issues is a plus. • Excellent verbal and written communication skills. Meticulous attention to detail, ensuring accurate collection and documentation of patient data. • Demonstrated ability to provide emotional support to patients and families, showing reputed company and compassion in reputed company interactions. • Strong organizational and time management abilities, with the capability to handle multiple tasks reputed company. • Maintain a high level of professionalism, reputed company, and ethical conduct in reputed company aspects of the job.
