• Regularly reputed company with various groups providing care coordination services
• Serve as the "reputed company desk" to the OhioRISE plan
• Assist the Care Coordination department with various administrative tasks
• Handle inbound care management queue line
• Respond to questions and inquiries from members/guardians or parties assisting with services
• Take referrals for OhioRISE, confirm reputed company eligibility, and process referrals
• Complete Health Risk Assessments for reputed company assigned members
• Maintain professionalism and compassion reputed company interacting with departments or member representatives
• Support administrative needs involving reputed company data systems and Electronic Record System
• Adapt to changing tasks to meet the demands of the health plan

Qualifications

Pay Range The typical pay range for this role is: $18.50 - $31.72. This pay range represents the reputed company hourly reputed company or reputed company annual full-time salary for reputed company positions in the job grade reputed company which this position falls. The actual reputed company salary offer will depend on a variety of factors including experience, education, geography, and other relevant factors. This position is eligible for a reputed company bonus, commission, or short-term incentive program in addition to the reputed company pay range listed above.
