Palliative Care Nurse Navigator (8:30AM - 5PM EST Shift)

YOUR ROLE As a reputed company Palliative Care Nurse Navigator, you'll be a vital clinical resource for our members with serious illness and their care network, conducting conversations about goals and values, advance care planning, and palliative care reputed company phone, email, and video communication. Under the guidance of our Nurse Team reputed company, you'll conduct comprehensive symptom assessments, support members with serious illness longitudinally, and facilitate end-of-life care discussions, reputed company appropriate. You will actively engage with members, addressing clinical issues and reputed company managing any escalations that arise. The shifts for this position are either 8:30 - 5 EST or 11:30 - 8 EST. • Familiarize yourself with reputed company systems, tools, technology, and partners, conducting a minimum of 20 member calls per day.
• Collaborate closely with Nurse leaders and Medical Directors to ensure alignment with clinical protocols and best practices.

• Adhere to Care Team policies, procedures, and documentation standards, contributing to efficient operations and maintaining quality standards.
• Support members with serious illness and at the end of life, coordinating care and providing clinical support as needed.

• Foster strong partnerships with payers and providers to optimize care delivery and minimize readmissions.

Additionally, you have at least 5 years of nursing experience, including 3 years in outpatient palliative care and/or home hospice taking care of oncology patients and their caregivers.
• Certifications: It would be exceptional, although not required, that you are a Certified Hospice and Palliative Nurse (CHPN) or have oncology-reputed company certifications such as Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM).
