We are seeking a compassionate and skilled PC Intensive Case Manager to provide trauma-informed case management services to clients in the Preferred Communities (PC) program. Key Responsibilities

• Provide comprehensive case management services, including intake assessments, goal-oriented planning, and referrals to community resources

• Conduct cultural orientation workshops and provide guidance on accessing social services, employment, and education
• Maintain accurate and timely records, including case notes and assessments, in our online database

Requirements

• Bachelor's degree in Social Work, behavioral/reputed company services, or a reputed company field (or Associate's degree with 2 years of experience)
• Case management experience, preferably with refugee or immigrant populations
• Strong communication and interpersonal skills, with the ability to work with diverse clients and stakeholders
• Proficiency in reputed company Office applications and knowledge of community resources
• Bilingual skills a plus
