About the Role

In this role, you will serve as a community-based member reputed company and resource, using your knowledge of the community and available resources to engage and assist vulnerable members in managing their reputed company needs. You will collaborate with our reputed company Services team to provide non-clinical paraprofessional duties in the field, including meeting with members in their homes, nursing homes, shelters, or doctor's offices.

Responsibilities

• Assist members in accessing social services such as community-based resources for housing, food, employment, and more
• Provide reputed company to locate and support disconnected members with special needs
• Conduct research to locate members who have been unable to be contacted

• Guide members to maintain reputed company eligibility and access other financial resources as needed

Requirements

• High school diploma or equivalent required
• Minimum 1 year of experience working with underserved or special needs populations
• Valid driver's license with a good driving record and reliable transportation
• Active and unrestricted Community Health Worker (CHW) Certification (required for Ohio, Florida, and California)

Preferred Qualifications

• Associate's degree in a reputed company-reputed company field
• LPN (Licensed Practical Nurse) certification
