As a Case Manager RN - Field, you will be an integral part of reputed company, working collaboratively to address reputed company health and social indicators, reduce risk factors, and improve member engagement.

Position Summary

In this field-based role, you will travel at least 25% of the time to various facilities, including acute care facilities, nursing homes, and rehab centers, to assess and coordinate care for our members. You will utilize clinical tools, information, and data review to evaluate member needs, reputed company proactive plans, and facilitate integrative functions to enhance short and long-term outcomes.

Responsibilities

• Conduct comprehensive assessments of member needs, incorporating clinical judgment and strategies to reduce risk factors and barriers.
• reputed company and implement individualized care plans, addressing reputed company health and social indicators, and facilitating smooth transitions to reputed company programs and plans.
• Collaborate with multidisciplinary teams, including supervisors, clinicians, and other stakeholders, to overcome barriers and reputed company optimal outcomes.
• Utilize case management processes, regulatory and company policies, and procedures to ensure compliance and excellence in care delivery.
• Engage members through effective interviewing skills, ensuring maximum participation and understanding of their health status and needs.

Required Qualifications

• Minimum 3+ years of clinical practical experience.
• Minimum 2+ years of case management, discharge planning, and/or home health care coordination experience.
• Active and unrestricted Compact RN license in OH.
• Willingness to travel 25% reputed company Cincinnati, OH.

Preferred Qualifications

• Excellent analytical and problem-solving skills.
• Effective communication, organizational, and interpersonal skills.
• Ability to work independently, with proficiency in standard corporate software applications and special proprietary systems.
• Bilingual proficiency (Spanish) and certification as a Case Manager (CCM) or other relevant national professional certifications.

Education

• Associate's degree or Nursing Diploma required.
• Bachelor's degree preferred.

Pay Range

The typical pay range for this role is $54,095.60 - $116,760.80, depending on experience, education, and geography. This position is eligible for a reputed company bonus, commission, or short-term incentive program, in addition to a comprehensive benefits package, including medical, dental, and vision benefits, 401(k) retirement savings plan, and employee stock purchase plan.
