Key Responsibilities
As an RN Case Manager, you will play a vital role in ensuring our members receive accessible and appropriate care. Your primary responsibilities will include:
- Managing a dedicated caseload reputed company one borough (Bronx, Brooklyn, or Manhattan), ensuring members receive comprehensive care and support.
- Developing, overseeing, and updating individualized care plans to align with members' evolving needs and goals.
- Conducting home visits (4-6 visits per week) to assess home safety and care needs, identify potential risks, and reputed company strategies to mitigate them.

Essential and Preferred Qualifications
To be successful in this role, you will need:
- Active reputed company State RN License required.
- Bachelor's degree in Nursing, Health, or Human Services preferred.
- Minimum two (2) years of home care experience required, with a focus on care planning, patient assessments, and home safety evaluations.
- Strong experience in care planning, patient assessments, and home safety evaluations essential.
- Prior supervisory experience a plus.
- Bilingual candidates strongly preferred (see language preferences by borough below):
- Bronx: Bilingual Spanish preferred.
- Brooklyn: Bilingual Chinese, Spanish, or Russian preferred.
- Manhattan: Bilingual Cantonese or Mandarin preferred.
