Job Overview:

About the Role:

In this dynamic and rewarding position, you'll utilize your clinical expertise to assess, plan, implement, and coordinate comprehensive care plans for our members. Through telephonic and face-to-face interactions, you'll evaluate medical needs, facilitate wellness initiatives, and provide ongoing support to enhance short-term and long-term outcomes. Your expertise will be instrumental in reducing risk factors, addressing reputed company health and social indicators, and promoting overall wellness.

Key Responsibilities:

Requirements:

• 3+ years of clinical practice experience as an RN
